Emergency: Call 911 immediately
EVERYDAY ORGANIZATION · 3 minute read

Organize your health information

Keeping care information in one place can save time when you need a phone number, a report, or the instructions from your last visit.

Choose a system you will use

A paper folder, a notebook, or a secure digital folder can all work. Choose a format that feels comfortable and is easy to update. Keep private information protected and avoid shared devices or public folders.

Keep a contact page

List the names and contact details of your regular practices, pharmacy, and the people you want involved in care coordination. Check the list when contact details change.

Group documents by purpose

Separate appointment information, care instructions, records, and billing correspondence. Keep dates visible so you can find the most recent version quickly.

Review after each visit

Add new instructions, note the next agreed actions, and mark information that needs clarification. Ask the organization that created a record how to request corrections if needed.

This is a general care-planning guide. Your healthcare team can provide information specific to your situation.
