Emergency: Call 911 immediately
PLANNING YOUR CARE · 3 minute read

Make the most of your appointment

A short list of notes can help you keep your appointment focused on what matters to you. You do not need perfect notes—just a few reminders.

Start with your main questions

Write down what you hope to discuss, then put your most important questions first. If you are bringing up a change you have noticed, note when it began and what you want the doctor to understand.

Bring the information you were asked for

Check your appointment instructions for documents or records to bring. Keep a current list of medicines and the contact details of other practices involved in your care. Ask the practice in advance if you are unsure what is needed.

Make communication comfortable

Tell the practice about any language or accessibility support you need. If someone is joining you, explain how you would like them to help during the conversation.

Leave with clear next steps

Before you leave, ask who to contact with follow-up questions and how any next appointments or results will be communicated. Write down the agreed actions while they are fresh in your mind.

This is a general care-planning guide. Your healthcare team can provide information specific to your situation.
